Neurological & Motility
Neurological evaluations
New positive Afferent Pupillary Defect (APD)
New Patients with different size pupils (ABR)
Strabismus/Diplopia – new onset (For ABR ok to Dilate if Diplopia is Monocular not Binocular)
Oculoplastic
Blepharochalasis
Ptosis
Ectropion/Entropion (any lid problems)
Note: Do not dilate when this is the primary reason for visit or if these are new findings. Dilate when long standing diagnosis and here for an annual exam.
Nasolacrimal Duct Obstruction (NLDO)
Dacryocystitis & Epiphora
Cornea
Corneal tumor – near limbus
Corneal sensitivity evaluation
Bandage Contact Lens present (ABR use I-Care)
Post-op Pterygium or Pingueculum
Referral for possible Pupilloplasty
Any post-op Pupilloplasty
LASIK, LASEK, PRK post-op – see LASIK preference NYC only – (Dilate after 6 months P/O)
Keratitis
RES patient
New Pterygium patient
Keratoconus if they had PK (unless IOL present) (ABR dilate all)
Infection
Acute Red Eye
New Iritis
Infection
History HSV, HZO or Neuro issues
Uvea (Iris, Ciliary body, choroid)
Ciliary Cyst
Iris Nevus (if new lesion or referred as suspicious lesion) can dilate if it has always been there and flat)
New Iritis
Glaucoma
High Hyperopes - unless had PI or cataract surgery
Increased IOP > 40
Angle closure unless had PI
Pigmentary Glaucoma
Angle Recession Glaucoma
Retina
Pigment Epithelial Detachment (PED) especially in diabetics
Central Vein Occlusion if diagnosis within last 6 months – TPW only (BRVO & CRAO OK to dilate)
Visual Fields
Visual Fields unless pupils < 3 mm (Never Dilate for MGE)
Other
Trauma
1 day S/P CSM
Dislocated IOL
When in doubt

do not dilate for:
Uvea (Iris, Ciliary body, choroid)
Iris Nevus (if new lesion or referred as suspicious lesion) can dilate if it has always been there and flat)
